Most adults do not arrive with a tidy single problem. Anxiety and sleep and work stress and a strained relationship tend to show up together, and splitting that across a prescriber at one practice and a therapist at another means nobody sees the whole picture.

Coordinated rather than parallel
When your prescriber and your therapist are in the same practice, a medication change can be informed by what is actually happening in therapy, and therapy can account for what a medication is or is not doing. That coordination is the practical argument for keeping both in one place.
Common reasons adults come in
Persistent worry, low mood, difficulty concentrating, sleep that has stopped working, grief, burnout, and relationship strain. Some people want therapy only. Some want a medication consultation only. Both are fine, and you are not signing up for a package.
